Enrollment Checklist
We are pleased that you are considering Compass Charter Schools as part of your child's educational future. Follow this Enrollment and Registration Checklist to review the required documents.
Enrollment and registration checklist
Enrollment at Compass is quick and easy when families follow the Enrollment and Registration Checklist.
Enrollment checklist
1. Scholar's proof of age
Submit a copy of one document that verifies the scholar's age.
Accepted examples
- U.S. or foreign birth certificate
- Passport
- Baptismal or adoption record
- Parent/legal guardian or pediatrician affidavit
- Other U.S. or foreign government-issued identification
If none is available, contact Enrollment to discuss alternatives. Families may order California birth records through the California Department of Public Health or find another state's vital-records office through the CDC.
2. Two proofs of residence
Submit two documents showing the parent/guardian name and current residential address. Utility and similar statements must be dated within the past 45 days; property-tax bills must be dated within the last year.
Accepted examples
- Residential utility, cable, telephone, or cellular bill
- Deed, property title, mortgage statement, or current property-tax bill
- Signed rental/lease agreement, Section 8 agreement, rental contract, or rent receipt
- Government correspondence, voter-registration notice, court document, or USPS change-of-address confirmation
- Bank, credit-union, credit-card, insurance, IRS, or California Franchise Tax Board record
- California vehicle/vessel title or registration
- Letter from a shelter, nonprofit, employer, faith-based organization, or government agency confirming residence
- Caregiver affidavit or declaration of residency, when applicable
Your county determines the Compass charter school serving your family. See the current county list on Enroll Now. If you cannot provide two listed documents, contact Enrollment for acceptable alternatives.
3. Immunization record, if available
Compass' Online and Options programs are independent-study programs with no classroom-based instruction. Under current California guidance, scholars in this setting are not required to provide immunizations for school entry. If your scholar has an immunization record, please provide it so Compass can maintain complete health records.
Learn more from the California Department of Education's immunization guidance.
4. Health examination and oral health assessment
Each scholar enrolling in kindergarten, or entering a California public school for the first time, must have a completed Health Examination and Oral Health Assessment Form.
Parents may waive either requirement by completing the applicable waiver. Assessment and waiver forms are available through an Enrollment Specialist.
5. Prior academic records
- Grades K-8: a copy of the report card from the most recent school year.
- Grades 9-12: an official transcript with complete end-of-year grades from the prior school year and any academic documentation issued for the current school year, such as a progress report, class schedule, or marking-period/semester report card.
- Previously homeschooled, grades K-8: a Prior Academic History Report Card showing completed courses and the completed school year.
- Previously homeschooled, grades 9-12: records including evidence of attendance, state or national test results, curriculum followed, a copy of the Private School Affidavit, and a portfolio of work for review.
Request records from the scholar's previous school or district. If you cannot obtain them, contact Enrollment for help.
6. Additional documents for special circumstances
- Proof of legal guardianship or applicable court order
- Disciplinary reports if the scholar was expelled or suspended within the last year
- Most recent Individualized Education Program (IEP) from within the last year
